ALM in RAC - Workshop 1 | Print |

 

Workshop 1 is a two-hour session, which addresses communication between GPs and aged care homes, and coordination of the residential aged care MBS items. Particularly, identifying organisational systems and tools that facilitate communication and integration of medical care.

The workshop is divided into four components: introduction, communication, medical care and summary.

  • Introduction: short presentation
  • Communication: presentation and activity. Activity highlights various tools for improved communication and coordination of patient care
  • Medical care (MBS items): presentation and case study activity. Activity highlights how GPs and aged care home staff can work together on MBS items
  • Summary and next steps: short presentation

A Running Sheet is provided, which details a suggested timeline for the workshop. Use the running sheet to plan the session with your co-facilitator. In the right-hand column nominate the person responsible for each action.

The Running Sheet also lists the resources and handouts to provide to participants (as part of a workshop pack).

The Presentation for workshop 1 is provided in Microsoft PowerPoint, and includes detailed notes for each slide. Go through the presentation and determine which slides you will delete and alter. Adapt the notes into your own words.

There are two activities in workshop 1. Activity 1 addresses communication, whilst activity 2 focuses on the residential aged care MBS items. Instructions for each activity are provided.

At the completion of the workshop instruct participants on what happens next in the ALM. Disseminate the handouts on the private learning / homework that participants must complete before the second workshop (Handouts are provided). Also, restate the date of the second workshop.
